From the Ground Up: Why Gait Matters

Walking is something most of us do without thinking. Each step involves a carefully coordinated sequence of movements through your feet, ankles, knees, hips, pelvis and lower back.

When one area becomes painful or stiff, your body naturally adapts. You may take shorter steps, spend less time on one leg, lean your body or change the way your foot meets the ground. These changes can be helpful in the short term because they protect a sore joint.

Over time, however, a new walking pattern can become familiar. The way you move may continue to place extra demand on other parts of your body, even when you are no longer consciously trying to protect the original source of pain.

Every step creates a pattern of loading

When your foot meets the ground, your body absorbs and manages force. This is known as loading.

Your knee, hip and ankle share the work of supporting your weight, controlling your movement and helping you push forward into the next step. Your muscles and joints work together to keep you balanced and moving efficiently.

Loading is not automatically harmful. Your body is designed to manage the forces involved in walking. Problems can arise when the load is repeatedly concentrated in one area, when one side of the body does more work than the other, or when movement is restricted by pain or stiffness.

For example, knee pain may lead you to place less weight through that leg. You may shorten your stride or move quickly over the painful side. This can reduce the time your knee is working, but it also changes how your other leg, hip and lower back contribute to walking.

The issue is not simply how much you move. It is how the work is shared as you move.

Pain can change the way you walk

Pain often encourages protective movement. This is a normal response from the nervous system.

If putting weight through your right knee is uncomfortable, you might:

  • Take smaller steps
  • Keep your right knee slightly bent
  • Spend less time standing on the right leg
  • Push off more from the left foot
  • Lean your upper body to change the pressure through the knee

These adjustments may happen gradually. You might not notice them until someone points out a limp, an uneven stride or a change in your posture.

Hip pain can create a different set of adaptations. Some people lean towards the painful side to reduce the effort required from the muscles around the hip. Others alter their pelvic movement or shorten their stride.

When pain affects the lower back, walking may become more rigid. You may reduce the natural movement of your pelvis, rotate your body differently or avoid fully extending through your hips.

These patterns are not signs that you are walking “badly”. They are your body’s attempt to keep you moving while managing discomfort.

Compensation: helpful at first, tiring over time

A compensation is a change in movement that helps one part of the body cope with a problem elsewhere.

Imagine carrying a heavy bag in one hand. You may lean slightly to the opposite side to keep your balance. You can still walk, but your posture and muscle activity have changed.

Walking with joint pain can produce a similar effect. If one knee is doing less work, the other leg may take on more. If your hip is stiff, your pelvis or lower back may move more to help your leg travel forward. If your ankle is not moving freely, your knee or hip may compensate during the next stage of the step.

This does not mean that every compensation will cause another painful condition. Bodies are adaptable, and people move in many different ways. But repeated changes in loading can contribute to fatigue, stiffness and discomfort in areas that were not originally painful.

That is why someone with knee pain may also notice hip or lower back symptoms. The different areas of the body are connected through movement, balance and load.

It is more than posture

Gait is not just about whether your feet point forwards or whether you stand upright. It includes the timing, direction and amount of movement at each stage of a step.

Important features include:

Step length
A shorter step may reduce discomfort, but it can also change the rhythm and effort of walking.

Stance time
This is the period when your foot is on the ground. Spending less time on a painful leg can shift more work to the opposite side.

Foot placement
Where your foot lands affects how forces travel through your ankle, knee, hip and trunk.

Knee and hip movement
The amount of bending and straightening helps your body absorb force and move forwards.

Pelvic and trunk movement
Your pelvis and upper body help maintain balance. Changes here can influence the demand placed on the lower back.

Two people may walk at the same speed and appear to have similar strides, while using their joints and muscles in very different ways. A closer assessment can reveal movement patterns that are not obvious at first glance.

When pain has been present for a long time, simply being told to “walk normally” is rarely enough. Movement needs to be understood in context.
